In collaboration with the Colorado Department of Public Health and Environment (CPHE), The Southern Ute Incident Management Team and the Southern Ute Indian Tribe will be offering COVID-19 Moderna vaccinations to community members who reside within the Southern Ute Health Center Service Area on March 25, 2021 from 9:00 a.m. – 3:30 p.m. Priority will be given to the following:
- Enrolled Southern Ute tribal members & household members
- IHS-beneficiaries & household members,
- Southern Ute tribal staff & insured dependents

- Individuals must check-in at the testing event located at the hotel entrance
- Face masks are required.
For questions, you can reach the Southern Ute Indian Tribe COVID-19 Call Center Monday through Friday from 8:00 a.m. to 5:00 p.m.
Deadline for registration will be 5:00 p.m. on March 22, 2021
There is also opportunity to sign up for a COVID-19 vaccination at the Southern Ute Health Center by appointment. Health Center staff can be reached at (970) 563-4581. The following dates and available COVID-19 vaccination doses are listed as follows:

3/2212021 – 43 Moderna
3/23/2021 – 8 Pfizer
3/25/2021 – 17 J&J
3/26/2021 – 36 Moderna
